2008-07-05 How effective are dental referrals by primary care physicians

The investigators report their results today during the 86th General Session of the International Association for Dental Research. Of the 24,403 children in the study, about 5 pct. had tooth decay, 2. 8 pct. were referred to a dentist, and 3. 5 pct. made a dental visit before 42 months of age. Among children with tooth decay, 32 pct. were referred, and a higher percentage of children with a referral visited a dentist (35. 6 pct. ) than those not referred (12. 0 pct. ). Several factors were associated with whether a child received a referral to a dentist besides the presence of any decayed teeth, including increasing age of child, the availability of a general dentist in the county where the child received IMB services, and whether the child was seen in a practice located in a rural area.

Young children from low-income families experience high levels of tooth decay and face many barriers to getting dental treatment and preventive services. Because these children usually visit their pediatrician or other primary care provider far more frequently than a dentist, the primary care medical setting is gaining popularity as a place to provide preventive dental services.

I moved to the United States in 2000, and the treatments available through the insurance based system here makes the British dental system look primitive by comparison. Few people here have dentures, even those over 70, and there is a lot of periodontal care that I had never come across in the UK. I had gum problems that were never treated by my NHS dentists and the first thing that my new American dentist asked me when I went for my check up was, if I had seen a dentist regularly while I lived in the UK. The answer was that I had gone regularly, but never had a dentist that even had a dental hygenist. No dentist over here is without a dental hygenist, and they make you feel like a client.

Too often, affordability and other barriers keep Maine people away from the dentists chair and stuck with easily preventable problems. With current energy costs, many families may postpone or choose not to have regular dental care. Getting treatment by local dental professionals is more important than ever because driving many miles for a dental appointment is no longer so easy.

In April, the Legislature unanimously approved a new license option for dental hygienists enabling them to set up their own practices without a dentists supervision. After meeting rigorous license requirements, independently practicing dental hygienists can perform specified procedures on their own and are required to refer patients in need of additional care to a dentist.
